Orbital subperiosteal hemorrhage is rare, with most cases occurring in young males as a result of direct facial or orbital trauma. The authors present a case of a spontaneous subperiosteal hematoma of the orbit in a 4-year-old girl.
Background: Subperiosteal hematoma is the accumulation of blood between the periosteum and the skull, which commonly occurs in the neonatal period but rarely in other ages. Subperiosteal hematoma can be self-absorbed in most cases and occasionally may cause ossification.

The risk of compressive optic neuropathy may be higher in patients with bilateral hematoma and massive subgaleal hematoma.
Subperiosteal hematoma of the orbit is a rare but well-recognized entity, usually caused by trauma. Two cases of subperiosteal hematoma associated with sinusitis are presented. The risk of compressive optic neuropathy may be higher in patients with bilateral hematoma and massive subgaleal hematoma. Most patients are treated with evacuation of the hematoma. Subperiosteal haematoma of the iliac bone is an extremely uncommon clinical entity, however nonetheless can cause leg pain and weakness, most commonly in adolescent patients with a recent history of sporting injury or other trauma. Many of these lesions spontaneously resolve. However they may also become chronic and ossify. Orbital subperiosteal hematomas (OSPHs) are less common; occurring primarily in young males as a direct result of craniomaxillofacial trauma, although the development of spontaneous OSPH has been reported. The etiologies of OSPH may be divided into 4 subcategories: congestive, traumatic, spontaneous, and systemic.
